Call for Nominations: 2023 Disability Services Awards

Student Life Disability Services invites the university community to submit nominations for two awards: The Outstanding Student Advocate Award (for students) and the Dick Maxwell Award (for faculty/staff). Both awards recognize an exemplary commitment to disability access and inclusion on campus.
.Nominations are due Friday, April 14th.
How to Submit a Nomination
Please submit a nomination narrative (Word doc format, 1 page maximum) to slds@osu.edu by Friday, April 14th. In your nomination, please include:
- The name of the award for which you are submitting a nomination
- The name, email address, and campus address (if available) of your nominee
- Your relationship to the nominee
- Specific examples of the nominee's contributions to access and inclusion on campus. See the award descriptions below for specific criteria.
Outstanding Student Advocate Award
The Outstanding Student Advocate Award will be presented to an OSU student (undergraduate, graduate, or professional) who has been an exemplary advocate for disability access and inclusion. Nominations may come from any student, faculty, or staff member.
Eligibility
All full-time or part-time students at The Ohio State University are eligible to receive this award.
Award Criteria
The award recipient will be selected for the following kinds of contributions:
- Significantly improving or enhancing the access or inclusion of individuals with disabilities, either on campus (preferred) or off campus.
- Contributing to outstanding and sustained improvement in the services provided by the Office of Student Life Disability Services, or their department in respect to students with disabilities.
Dick Maxwell Award (Staff or Faculty)
The Dick Maxwell Award will be presented to a staff or faculty member who has made an outstanding contribution to a student with a disability or to Student Life Disability Services.
Eligibility
All full-time or part-time faculty and staff members at The Ohio State University are eligible to receive this award.
Award Criteria
The Award recipient will be selected for the following kinds of contributions:
- Significantly improving or enhancing the quality of the higher educational experience in ways that make a substantial difference for a student with a disability.
- Contributing to outstanding and sustained improvement in the services provided by the Office of Student Life Disability Services, or their department in respect to students with disabilities.
