Documentation Guidelines

Disability Services reviews documentation to understand how a disability affects a student in academic and cocurricular settings and how it relates to requested accommodations. Each request is reviewed individually and as part of an interactive process. 

Documentation should confirm a diagnosis or show prior accommodations. Examples include: 

  • IEP or 504 Plan
  • Medical records or patient portal summaries
  • A signed letter from a healthcare provider confirming a diagnosis
  • Accommodation letter from a previous college or university
  • Full evaluation or diagnostic report (such as an ETR or MFE) 
  • Disability Services Verification Form

Important to Know

  • Documentation and applications are reviewed in the order they are recieved.
  • Submit documentation as early as possible to avoid delays.
  • Notification of next steps will occur via email within 3-5 business days of reciept of a complete application.

Documentation Requirements

Informed by guidance from the Association on Higher Education and Disability (AHEAD) on documentation review and accommodation approval.  

Academic Accommodations

Students requesting academic accomomdations must complete the appropriate application in AIM.

Please include:

  • Documentation (as described above) indicating diagnosis and linking diagnosis to accommodation request.
Housing Accommodations

Students requesting housing accommodations must complete the appropriate application in AIM. 

Please Include:

Note: Housing requests cannot be reviewed until all required documentation is submitted. 

Service Animals

Service animals do not require approval from Disability Services and students must notify Housing and Residence Education. Learn more about the difference between an Emotional Support Animal and a Service Animal.  

Observable Disabilities

Students with visible, permanent disabilities (such as wheelchair or white cane users) do not need to submit documentation. Instead:

  • Submit the SLDS Application
  • Call our office at 614-292-3307 or visit during drop-in hours.
  • Schedule your Welcome Meeting
Provisional Accommodations

In some cases, provisional accommodations may be approved for one semester while a student is completing the diagnostic process. Submit the SLDS Application to get started.

LD/ADHD Testing

To find local providers of LD/ADHD testing, visit the Counseling and Consultation Services Community Provider Database. Under Treatment Focus, select "Psychoeducational Evaluations".
