How to Register

This section describes how to register with Disability Services. Please select the description that best describes you and complete all the required application and documentation guidelines. If you have questions about which application process to follow, please call our office at 614-292-3307.

*After saving your application draft, you will advance to the ‘Files’ page where you can upload your SLDS documentation (you can log back into your saved application later to upload documentation if needed). Students with readily observable disabilities (e.g. wheelchair user, white cane user) do not need to submit documentation. Instead, please contact SLDS at 614-292-3307 after submitting your application.

Important: Your application is not complete and cannot be processed until you have uploaded your disability documentation to your application. Once your application is complete, you can expect to hear back from SLDS within 3-5 business days.
